Purpose and Administration of the Questionnaire
The Parties have agreed to administer the Questionnaire to a randomly selected sample of approximately 2,500 out of a total of more than 400,000 claimants. Responses to the Questionnaire will help inform and facilitate development of a global settlement matrix that may be used to value claims for the broader claimant population. While there is no broad settlement currently, the Questionnaire – and responses to it – are on the critical path to a resolution for the Parties.
Please note that completing the Questionnaire does not guarantee that a claim will be eligible for settlement. Failing to complete the Questionnaire per the instructions herein and by the date stated above, however, may result in any potential settlement being denied or substantially delayed by the Department of Justice, or an administrative claim being denied by the Department of the Navy.
Note that if you will be unable to meet the deadline of November 24, 2025, please contact the Settlement Masters at the email address above or, alternatively, at the contact number above by no later than November 10, 2025, and provide a good cause justification. Failure to do so may result in any potential settlement being denied or substantially delayed by the Department of Justice, or an administrative claim being denied by the Department of the Navy.
Steps To Complete the Questionnaire
All Questionnaires must be completed in PLG’s database called Rubris, PLG’s technical partner, which will collect and store all Questionnaire responses and related documents. The Questionnaire will be made available online in Rubris’s “Crosslink” portal. Claimants and/or their authorized representatives should complete the Questionnaire in that portal.
Upon receiving notice that the Rubris portal has been activated for completion of the Questionnaire, you may access the portal using your existing Rubris Crosslink account, preview the questions, and begin completing the Questionnaire. If you do not yet have a Rubris Crosslink account, contact their support team at crosslinksupport@rubris.com to request access.
Once you start the process of completing the Questionnaire in Rubris, you will have the option to save your work, exit and then restart at a later date using the same login credentials.
Upon completion of the Questionnaire, the claimant will be asked to affirm that he or she has answered all questions to the best of his or her ability. Once he or she has verified the answers to all questions, you should click “complete” [“finished”]. Once you have clicked “complete” [“finished”], your work will be saved but you will no longer have the ability to change, modify, add or delete any information entered into the Questionnaire. Please be sure that you have fully completed all aspects of the Questionnaire prior to hitting “complete.”
By creating an account in Rubris and submitting a Questionnaire through Rubris, you and your client consent to sharing submitted information and any related documentation with the PLG, the United States, and the Settlement Masters.

The Questionnaire is comprised of two primary sections:
- a “core” section which asks about the claimant’s background, time on base, medical history, and damages; and
- a “disease inserts” section, which asks specific questions about the particular injuries this party is claiming.
You should complete disease inserts for each relevant disease that the party is claiming.
Although the Questionnaire has been attached here as a preview, the Questionnaire can only be submitted online through Rubris in order to be considered in this matrix-building process.
Documentation
Responses to specific Questionnaire questions should be supported by documentation, where available and applicable.
To support your responses, you can obtain military personnel records directly from the National Archives and Records Administration (NARA) either via eVetRecs (available at https://vetrecs.archives.gov) or by manual submission of Standard Form 180 (SF-180) (available at https://www.archives.gov/veterans/military-service-records/standard-form-180 – SF-180).
You may also obtain health care records directly from the claimant’s medical treatment facility.
If the claimant is living and responding to the Questionnaire for themselves, he or she may obtain a Social Security Statement online by creating an account at login.gov, then using this account to sign-in to ssa.gov. Once signed into ssa.gov using the login.gov account, he or she can download their Social Security Statement. Similarly, he or she can obtain their Department of Veterans Affairs (VA) benefit records by creating an account at login.gov, then using this account to sign-in to va.gov. After creating an account at login.gov and signing in at va.gov, a claimant may also download his or her VA Benefit Summary Letter. Claimants may also be able to review, print, and download some of their VA medical records online using their login.gov account to sign-in to https://www.va.gov/health-care/manage-health. Like private healthcare facilities, claimants will need to contact their VA healthcare facility directly to get a complete copy of their VA medical records. If documentation is not provided to support your responses, the DOJ may take the position that those supporting documents do not exist. Failure to provide supporting documentation may also affect the potential overall values of any global settlement matrix.
The claimant must also submit an affirmation attesting to their responses to the Questionnaire. The United States may audit your client’s responses to determine their accuracy and truthfulness and take any and all action deemed necessary under federal law.
Effect on Settlement
Claimants selected to complete the Questionnaire remain eligible for Elective Option offers, provided their claims otherwise satisfy the Elective Option’s requirements. The Elective Option is a voluntary framework that has been adopted by the DOJ and DON to evaluate and potentially settle certain claims brought under the CLJA. The United States shall review completed, documented Questionnaires for eligibility under the Elective Option. Again, completing the Questionnaire does not guarantee that your claim will resolve through settlement or the Elective Option.

Rubris will make the Questionnaire available on the Crosslink platform by September 25, 2025. If you have already uploaded the claim for a client to Rubris, please send an email to crosslinksupport@rubris.com with the claimant’s name, their CLCU claim ID (the “Navy Portal ID”), and the Rubris Crosslink ID for the claim you uploaded for them. If more than one of your firm’s clients has been selected to complete a questionnaire, please assemble those 4 data points (First name, Last Name, Navy Portal ID, Crosslink ID) for all of your firm’s selected claimants in a single email to crosslinksupport@rubris.com. If you have not yet uploaded the claim to Rubris, please contact crosslinksupport@rubris.com immediately so that they can set you up.
At some point in the future, you may be asked to sign an authorization to allow additional information to be reviewed by the government.
